Annette Pratt
PMHNP | Provider
Hi, I’m a Psychiatric Mental Health Nurse Practitioner with over 18 years of experience in mental health and addiction nursing. I earned my Master of Nursing from the University of North Dakota and am currently completing my Doctorate there as well. My approach to care is trauma-informed, person-centered care with a strong emphasis on education and partnership - creating treatment plans that are tailored to each person’s specific goals. I strive to offer a safe, supportive space where healing can be honest, empowering, and even full of laughter. My greatest passion is helping people build meaningful lives—one step at a time. Outside of work, I love spending time with my family, playing games and cards, watching movies, getting my hands dirty in the garden, tackling DIY projects, and exploring Minnesota’s beautiful state parks whenever I can.

Mackenzie Snortum
PMHNP | Provider
I’m Mackenzie Snortum, a Board-Certified Psychiatric Mental Health Nurse Practitioner and the founder of Enlightenment Psychiatry. I’ve been in the healthcare field since the age of 16 and have been practicing as a PMHNP since 2020. I created Enlightenment Psychiatry with a vision to offer a modern and innovative approach to mental health care—one that breaks the mold and embraces the unique journey of each individual. I believe in thinking outside the box, using non-traditional methods, and holding space for self-guided, holistic healing. Whether you're joining me in person in Park Rapids, MN, or connecting through telehealth anywhere in Minnesota, my goal is for you to feel safe, supported, and empowered to share your story—without fear of judgment or pressure to conform. This is a place to be real, to heal, and to grow. Outside of work, I love anything outdoors, a good puzzle on a rainy day, and spending meaningful time with my beloved friends and family.

Cristin Trejos
LICSW, LADC
Hi, I’m Cristin Trejos, a Licensed Independent Clinical Social Worker (LICSW) and Licensed Alcohol and Drug Counselor (LADC). I provide psychotherapy to individuals ages 6 and up in an outpatient setting. My journey into mental health care began in 2008, when I felt a strong calling to support individuals and families navigating life’s challenges. Early in my career, I worked with Hubbard County Social Services in child protection, providing in-home support to help families meet their goals. I graduated from Bemidji State University in 2012 with a Bachelor of Social Work and a Chemical Dependency Certificate, then went on to work as an LADC in two outpatient treatment programs. I also gained specialized training and experience as a mobile crisis mental health practitioner. In 2020, I returned to school to earn my Master of Social Work so I could broaden my clinical scope and better serve those living with substance use and co-occurring disorders. My goal is to offer a compassionate, supportive space for healing—where people of all ages feel seen, heard, and empowered. When I’m not working, I enjoy traveling anywhere warm, spending time with my husband and children, playing a round of golf, and relaxing with a good classic movie.
