Bring Key Information

Insurance card and valid ID (we accept both insurance and self-pay).

List of all medications, including supplements and over-the-counter medicines.

Any recent lab results or psychiatric/therapy records. (we can always request these for you!)

Prepare Mentally and Emotionally

For Established Patients
Complete Your Forms

You’ll receive forms the night before your appointment for updates or annual consent forms.

Completing these forms ahead of time helps your provider understand how you’ve been doing since your last visit and allows your appointment to focus on your care.
Bring Any Changes in Information
Updated insurance or ID, new medications, or recent medical records.
Prepare for Your Visit
Note any changes in symptoms, concerns, or goals you’d like to discuss.
Prepare questions for your provider to make the session as helpful as possible.

Telehealth Visits

Make sure your device is charged, and your camera and microphone are working.
