-
Bring any questions you have for your provider.
Prepare Mentally and Emotionally
We understand that the first appointment can bring a mix of emotions. That's completely normal, and our team is here to support you and create a safe, welcoming space.

For Established Patients

Complete Your Forms
-
You’ll receive forms the night before your appointment for updates or annual consent forms.
-
Completing these forms ahead of time helps your provider understand how you’ve been doing since your last visit and allows your appointment to focus on your care.

Bring Any Changes in Information
Updated insurance or ID, new medications, or recent medical records.

Prepare for Your Visit
Note any changes in symptoms, concerns, or goals you’d like to discuss.
