PREPARING FOR YOUR VISIT

For New Patients

Bring Key Information

  • Insurance card and valid ID (we accept both insurance and self-pay).

  • List of all medications, including supplements and over-the-counter medicines.

  • Any recent lab results or psychiatric/therapy records. (we can always request these for you!)

 

Think About Your Goals

  • Consider what you’d like to focus on, such as symptoms, challenges, or treatment goals.

  • Bring any questions you have for your provider.

 

Prepare Mentally and Emotionally

​We understand that the first appointment can bring a mix of emotions. That's completely normal, and our team is here to support you and create a safe, welcoming space. ​

For Established Patients

Complete Your Forms

  • You’ll receive forms the night before your appointment for updates or annual consent forms.

  • Completing these forms ahead of time helps your provider understand how you’ve been doing since your last visit and allows your appointment to focus on your care.

Bring Any Changes in Information

Updated insurance or ID, new medications, or recent medical records.

​

Prepare for Your Visit

Note any changes in symptoms, concerns, or goals you’d like to discuss.

Prepare questions for your provider to make the session as helpful as possible.​

Type of Visit

In-Person

  • Please arrive 5–10 minutes early to allow for a smooth check-in and ensure your appointment starts on time.

 

Telehealth Visits

  • Make sure your device is charged, and your camera and microphone are working.

  • Find a private, quiet space for your session.

  • Log in 5 minutes before your scheduled appointment time.
